(collectively referred to as the “Zorlu Enerji Group”).

Pursuant to Law No. 6698 on the Protection of Personal Data (“KVK Law”), individuals whose personal data is processed — defined as “data subjects” under the law (hereinafter referred to as the “Applicant”) — are granted the right to make certain requests regarding the processing of their personal data, as set forth in Article 11 of the KVK Law.

According to the first paragraph of Article 13 of the KVK Law, any requests related to these rights must be submitted in writing to our Company, which acts as the data controller, or through other methods determined by the Personal Data Protection Board (“Board”).

Accordingly, any applications submitted to our Company must be made by completing and printing out this form and submitting it:

• In person by the Applicant,
• Via notary,
• By sending it to the Company’s registered electronic mail (KEP) address, signed with a secure electronic signature as defined in the Electronic Signature Law No. 5070,
• Via mobile signature,
• Or, if applicable, through the email address previously provided to and registered in our Company’s systems.
